Note:  This form should not be filled out for current students.  Only students new to the school should fill out this form.  Current students should fill out the returning student application and return it to Maggie at the NWIS office. 

Please complete  the entire application for each child applying to the school.

• Then, review the TIS/NWIS enrollment flyer. 
• Review the Which Learning Stage Do I Apply For document to determine which academic level will best meet the needs of your student.  
• Fill out the application below.
• You should receive an email confirmation after you press "submit".
• After you complete the application, please contact Maggie at lruiz@nearwestschool.org or call 216-961-4308 to schedule attendance at a Parent Information Session.
• Your application is not complete until you have attended a parent information session and provided copies of the required documentation (birth cirtificates, two proof of residency, custody documents (if applicable), and the $45 application/supply fee)
